[OCMEA] revised Scholarship form

Colleagues, I submitted an incorrect scholarship form to you yesterday, that also lacked adequate explanation, please allow me . . . This scholarship is through the Collegiate chapter of MENC at Syracuse University and is awarded to a senior(s) going into music education.  At the moment, there is not a designated amount on the form because the Chapter hopes to award as many scholarships as possible.  Our goal is to at least match last year's gifts of $250 each, but we may award more.  It would at least make a noticable dent in a semester's worth of book fees.
